Cognitive FX is a non-participating provider with any insurance company. This means we are not able to bill your insurance directly, but we can provide you with the necessary documentation to petition your insurance company for reimbursement following your treatment.

This also means that we do not have and cannot provide an NPI number; we have decided not to contract with any insurance companies therefore, we cannot bill your insurance directly. Many of the CPT Codes that are used during EPIC Treatment can be covered under many insurance plans.

Does Insurance Cover Treatment?

We do not bill insurance as a clinic, however, we can provide you with superbill for you to give to your insurance for possible reimbursement. The amount of reimbursement depends on your insurance plan. 

Is There a Payment Plan for Treatment?

Cognitive FX offers Care Credit as an option to help with payment for your treatment. 

Why don't you bill my insurance?

While insurance has many benefits, the ability of Cognitive FX to customize therapy to the individual and incorporate the most up to date research into therapy would be drastically reduced if we billed through insurance.

How will I know if I should do 1 week or 2 weeks of EPIC Treatment?

During you consultation, the practitioner will discuss which option would be best for you. Some factors they consider are your symptoms, your budget, and the severity of the injury.
