man filling out Cognitive FX online forms for complimentary consultation

Cognitive FX offers a complimentary consultation to all potential patients who suffer from a wide variety of things ranging from sports or vehicle injuries to general cognitive decline from other non-injury related factors. To schedule a consultation with one of our Neuroscience Practitioners, simply complete a set of forms online found here

Once we receive your paperwork, we will review it to ensure your situation is a good fit for our services. For example, we do not treat addiction. We want to make sure that we can help you get better based on your specific concerns and symptoms. We have helped individuals recover from concussions, neurological problems from lead poisoning, neurological problems from carbon monoxide poisoning, along with helping the brain to function better after stroke(s).

Once we get the paperwork reviewed, we will contact you to schedule your complimentary consultation. If you do not live close to the clinic, this will be done over the phone. Typically, consultations are a half an hour and are scheduled on Monday, Tuesday, or Thursday.

The goal of this consultation is to review your medical history and give you the information you need about EPIC Treatment. You will have the chance to ask us any questions you might have. During this consultation, we will also discuss different treatment options and the cost of EPIC treatment. This is determined by the recommendations and the choices you make during this consultation.

Overall, the complimentary consultation is to help you get answers to your questions and to help you better understand how we can specifically help you recover from lasting concussion symptoms. If you have any questions please do not hesitate to contact us at 385-375-8590 or by email here
